About Hilo

Hilo was founded in Neuchâtel in 2018, when Mattia Bertschi and Josep Solà — after nearly 15 years of research at CSEM (Swiss Centre for Electronics and Microtechnology) — founded Aktiia to commercialise the technology they had developed: continuous blood pressure monitoring without an inflatable cuff. The problem they tackle is ancient and underappreciated: the blood pressure cuff has not changed substantially since 1881, yet hypertension — which affects nearly half of US adults — is measured only sporadically, in clinical settings or through occasional home checks. Hilo Core combines three components: a wrist optical sensor using photoplethysmography to continuously estimate blood pressure, an FDA-cleared cuff for periodic calibration, and a mobile platform for longitudinal cardiovascular trend analysis. The system generates 30–35 readings per day — including overnight measurements — allowing users and clinicians to identify long-term patterns rather than isolated snapshots. In 2025, the company rebranded from Aktiia to Hilo and closed a $42M Series B, bringing total funding to $100M. In July 2026, it added a $19M extension led by Dell Family Office — the investment office of Dell Technologies founder Michael Dell — to fund the US expansion and the launch of Hilo Core. The technology is validated by eight clinical studies, used by more than 200,000 people globally, and received FDA clearance as an over-the-counter (OTC) device — the first cuffless blood pressure monitor to achieve this recognition.

The Story

Founded in 2018 in Neuchâtel by Mattia Bertschi and Josep Solà after ~15 years of CSEM research on optical blood pressure monitoring. Rebranded from Aktiia to Hilo in 2025.

How Hilo works

Business Model

Hardware sales (Hilo Core device) and software/analytics subscription; B2C model with emerging B2B pipeline for healthcare providers.

Revenue Model

Hardware sales + monthly app subscription; B2B pipeline with clinics and health systems in development.

Products & Services

Key Products

  • Hilo Core (sensore da polso ottico + cuffia FDA-cleared + app mobile)
  • Hilo App (piattaforma analytics cardiovascolari)

Core Use Cases

  • Continuous blood pressure monitoring for consumers with hypertension
  • Longitudinal cardiovascular trend analysis for doctors and patients
  • Nighttime blood pressure measurement (difficult with traditional cuffs)

Market & Clients

Key Customers

Targets adult consumers with hypertension or cardiovascular risk, and healthcare providers (physicians, cardiologists) seeking longitudinal blood pressure data on their patients.

Geographic Presence

How Hilo competes

Competitors

BiobeatValencellSamsung (Galaxy Watch BP)Apple (Research mode)Withings ScanWatch

Competitive Advantages

  • First FDA OTC clearance for a cuffless blood pressure monitor — difficult regulatory barrier to replicate
  • 8 clinical validation studies and 200,000+ global users
  • 15 years of CSEM R&D: deep and difficult-to-replicate IP

How Hilo grows

Growth Strategy

US launch with Hilo Core; expansion into B2B healthcare channels; subscription base growth.

Distribution Model

Direct-to-consumer online; potential pharma retail and healthcare channels (insurers, clinic networks).

Moat (Defensibility)

FDA OTC clearance, optical wrist sensor IP, proprietary clinical database with 200,000+ users.

Regulatory Context

OTC FDA clearance obtained (USA); CE mark obtained (Europe). Classified as a medical device, with implications for manufacturing, QMS, and post-market surveillance.

Key Risks

  • Competitive cuffless BP market with Big Tech (Samsung, Apple) entering.
  • Long and costly FDA pathway for any device update.

Strategic Insights

  • FDA OTC clearance is a significant competitive barrier: other players must replicate years of clinical validation.
  • Dell Family Office's investment signals interest from non-traditional VC capital in consumer medtech — different risk profile, long-term horizon.
